Care Manager RN - Per Diem

Seeking a Care Manager RN to lead patient-centered care coordination across the continuum. Responsibilities include developing safe discharge plans in collaboration with patients, caregivers, and healthcare teams, optimizing patient flow, reducing length of stay, and limiting readmissions through innovative, evidence-based practices. Qualifications include an Associate's Degree in Nursing, current RN license, and at least one year of hospital or acute care experience.
