What you’ll be doing Schedule: PRN, Monday‑Friday 8:30 am‑5:00 pm on site at our Chicago adult PHP facility. The Treatment Coordinator serves as the initial point of contact for patients, families, and the public; coordinates admissions and transitions; provides administrative support; and ensures timely completion of treatment plan assessments and patient engagement surveys. Essential duties and responsibilities Admissions and patient transitions – send welcome emails, identify referrals for care levels, coordinate with treatment team and providers, support patient admissions in the medical records system, register patients, scan documents, issue admission schedules. Administrative duties – greet visitors, obtain signed confidentiality statements, issue vendor badges, handle phone calls, maintain patient/family binders, schedule meeting spaces, keep breakrooms stocked, oversee mail, coordinate transportation, submit daily attendance logs. Program support – coordinate outside medical appointments for patients as directed by medical or nursing staff. Qualifications High school diploma or GED required. Bachelor’s degree in healthcare, business, or related field preferred. Compensation $18 – $27 USD per hour, based on experience and market expectations.
