The Community Health Worker (or Promotor at ¡Vamos! Health) plays a crucial role in promoting health and wellness among the patients we serve. This position works closely with community members to provide culturally competent health education, support, and navigation services that connect patients to the resources they need — from healthcare and social services to community-based programs. The CHW/Promotor acts as a trusted liaison between the healthcare provider and the community, guiding patients through the healthcare system, facilitating access to services, and addressing the social determinants of health that impact their well-being. Above all, the CHW/Promotor is committed to delivering an outstanding patient experience — building trust, showing up with empathy, and ensuring every patient feels supported, heard, and cared for at every step of their health journey.This role will report to the Community Health Lead.Essential Duties and Responsibilities:New Patient SupportCoordinate patient onboarding and account creationCoordinate and schedule appointmentsProvide assistance and/or referrals to help patients access other essential support servicesHelp patients understand medical instructions and connect with available resourcesServe as a liaison between patients and providers to ensure clear communication and understandingConduct SDOH and Needs Assessment surveysBuilding RelationshipsEstablish and maintain trusting, respectful relationships with patientsAdapt communication style to resonate with the community servedFoster a respectful and inclusive environment that values diversityServe as the key point of contact for patient needsProvide emotional support and encouragement to patients, fostering a sense of trust and safetyEducate patients about available health services and community resourcesHealthcare NavigationMaintain a strong understanding of the overall healthcare system, community resources, and ¡Vamos! Health services, and how they work togetherProvide patients with relevant resources and educationUtilize digital platforms to distribute educational materials, videos, and resources to patientsUpdate patient resource plans based on changing needs and circumstancesOrganize and lead virtual and in-person health workshops, webinars, and support groupsIdentify and address barriers to care promptly, developing solutions to improve access and quality of servicesData Collection & ReportingAccurately enter and update patient information, medical histories, and visit notes in designated systemsDocument patient interactions accurately and confidentially in accordance with HIPAA guidelinesCompile and report data on patient health trends, needs, and outcomesParticipate in regular team meetingsProvide ongoing insights and feedback to the Patient Care Lead regarding patient experiences or concernsOngoing Patient EngagementEngage in care planning and care plan managementFollow up on referrals to specialists and outside organizations by confirming appointment dates, ensuring patients have transportation, sending appointment reminders, and documenting the process as requiredResolve barriers to accessing resourcesManage ongoing communication with patients regarding membership, medical, and community-based needsComplete assigned certifications, training, and continuing education to strengthen patient engagement and health condition managementOther DutiesParticipate in regular meetings and training to stay informed on best practices and developments in community healthAttend training, workshops, and conferences to stay current on emerging trends in the fieldCollaborate with healthcare providers, clinic staff, and fellow CHWs/Promotores to ensure coordinated care and services for patientsParticipate in interdisciplinary team meetings to discuss patient cases and develop comprehensive care plansMaintain flexibility to work occasional evening and weekend hours as neededPerform other duties as reasonably assignedQualificationsBilingual proficiency in English and Spanish (written and spoken)Minimum of 3 years of experience in community health, community health education, social services, outreach, or a related fieldKnowledge of health promotion, disease prevention, and community resourcesStrong interpersonal and communication skillsAbility to work independently as well as collaboratively within a teamStrong critical-thinking and problem-solving skillsDeep understanding of and sensitivity to cultural nuancesAppreciation for and understanding of diverse cultures and communitiesStrong organizational skills and attention to detailCertification or training in community health work, public health, or a related field preferredEqual Employment Opportunity¡Vamos!
