Field-Based Housing Case Manager - Community Support
Community Connections, Inc.
Community Connections, Inc. is hiring a PSHP Case Manager in Washington, D.C. This role involves managing a case load of up to 25 residents, providing housing placement and supportive services. The successful candidate will work closely with clients to develop individualized plans and ensure they have access to necessary resources. The position requires a Bachelor’s degree in a related field and experience with mental health or substance use challenges.
