Clinical Pharmacist - Abdominal Transplant

Role Summary The Clinical Pharmacist works with physicians, care managers, and other clinical staff as part of a medical care team focused on patient outcomes and cost optimization. General Duties Patient Care Activities: Directs development, implementation, and maintenance of clinical pharmacy services; provides clinical consultation to the Medical Staff and other healthcare providers; suggests cost‑effective therapeutic alternatives; documents all activities and interventions; fosters teamwork; performs staff duties as needed. Collaboration and Education of Health Care Providers: Collaborates with all healthcare providers to resolve concerns; provides ongoing education and drug information to physicians, nursing, pharmacists, and other professionals via educational materials and in‑services. Development of Clinical Guidelines and Protocols: Facilitates the development and implementation of treatment guidelines, pathways, protocols, formulary changes, medication use evaluations, and drug information reports as approved by appropriate committees. Committee Membership and Regulatory Standards: Actively participates in clinical committees (e.g., pharmacy and therapeutics committee); acts as a liaison to other practitioners; demonstrates awareness of documentation, JCAHO, CMS, and State Board of Pharmacy standards; follows departmental policies and health‑system initiatives. Specialty Pharmacy – Accreditation Support: Ensures compliance with ACHC and URAC specialty pharmacy accreditation standards; participates in audit, quality improvement projects, and accreditation readiness activities. Prior Authorization Navigation: Leads and supports completion of prior authorizations and appeals; collaborates with providers, payers, and patient access teams; maintains current knowledge of payer policies and formulary restrictions. 340B Capture and Compliance: Ensures prescriptions align with 340B patient definitions, eligible providers, and covered entity sites; partners with the 340B operations team and documents clinical workflows impacting compliance outcomes. Education / Experience Qualifications Pharm.D. degree with 2 years of inpatient hospital/clinical experience or completion of a Pharmacy Residency. For specialty pharmacy roles: Pharm.D. degree with 2 years of retail/specialty pharmacy, hospital/clinical experience, or completion of a Pharmacy Residency. Licenses and Certifications Currently licensed or registered to practice pharmacy in Louisiana. Board certification in the applicable specialty area preferred.
