About the Role


We are seeking an experienced Physical Therapist (PT) - Skilled Nursing Facility (SNF) for a 13-week contract position in Santa Rosa, California. The Physical Therapist will evaluate and treat patients requiring rehabilitation services to improve mobility, strength, functional ability, and independence.


This role requires a patient-centered approach and close collaboration with nurses, physicians, occupational therapists, and other members of the interdisciplinary healthcare team.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on patient needs and functional goals.
• Provide therapeutic exercises, mobility training, and other physical therapy interventions to improve strength, balance, movement, and independence.
• Monitor patient progress and modify treatment plans based on clinical response and rehabilitation outcomes.
• Educate patients and families regarding therapeutic exercises, safe movement techniques, adaptive strategies, and ongoing rehabilitation needs.
• Maintain timely and accurate clinical documentation using Point Click and collaborate with the interdisciplinary team to coordinate patient care.

Requirements

• Active California Physical Therapist (PT) License Required.
• Minimum 1 Year of Physical Therapy Experience Required.
• Bachelor's, Master's, or Doctoral Degree in Physical Therapy From an Accredited Program Required.
• Skilled Nursing Facility (SNF) or Similar Rehabilitation Experience Preferred.
• Strong Knowledge of Physical Therapy Techniques, Therapeutic Modalities, Adaptive Equipment, and Patient-Centered Rehabilitation Required.

The Physical Therapist will provide individualized rehabilitation services focused on improving mobility, function, independence, and overall quality of life.
