Case Manager/Care ManagerCommunity Care is hiring a Case Manager/Care Manager to join our Family Care Program team. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Jefferson County position based in Walworth. Hybrid work available.Care Manager/Case Manager Job Responsibilities:Manages an assigned caseload of program members in collaboration with an RN Care ManagerCompletes comprehensive member assessments, member centered care plans, & functional screensProvides ongoing care coordination and quality assurance monitoring of servicesMaintains required documentation in adherence to established standards.Care Manager/Case Manager Job Requirements:· Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years' experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.· Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.· Certification: Eligibility for long-term care functional screener certification is required· Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.Care Manager Job Schedule:Monday to Friday, 8:00 AM-4:30 PMAbout Community CareCommunity Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community.
