The Nurse Care Manager works within the Behavioral Health Home (BHH) team to improve clinical outcomes for our clients. The Nurse Care Manager provides on-going member education, care coordination and collaboration with care providers and works to increase self-management support and timely healthcare delivery to achieve high quality care for members. The Nurse Care Manager acts as a member advocate for healthcare needs and works to ensure a continuum of care for all member needs including their physical and mental health. Requirements
RN with Maine State licensure.
Bachelor's degree
