Responsibilities
- Conduct comprehensive PT evaluations and re-evaluations for residents in a skilled nursing care setting.
- Develop and implement individualized treatment plans to address mobility, gait, balance, and activities of daily living.
- Deliver therapeutic exercises and modalities as indicated, applying evidence-based practices.
- Monitor resident progress, document outcomes, and adjust treatment plans accordingly.
- Collaborate with the interdisciplinary care team to support discharge planning and post-acute needs.
- Educate residents and caregivers on home exercise programs, safety strategies, and fall prevention.

Requirements
- Active physical therapy license to practice in Delaware (or eligible for licensure in Delaware).
- Degree from an accredited physical therapy program (DPT or equivalent).
- Experience in a skilled nursing facility or geriatric rehabilitation setting preferred.
- Ability to work collaboratively with an interdisciplinary team and adhere to documentation standards.
- Availability for a 13-week travel assignment with a 5x8 day shift schedule.
