Outpatient Oncology Clinical Pharmacist Specialist – Gilbert, AZ (Banner Health) Position Summary This position actively participates on the healthcare team as a pharmacotherapy expert to provide quality patient care in outpatient oncology. The pharmacist participates in optimal selection of drug therapy, designs regimens to maximize therapeutic benefit and minimize toxicity, and is accountable for ongoing evaluation and documentation of all components of pharmaceutical care delivered to patients. Core Functions Critically evaluate management of disease states and adjust therapy to improve patient medication regimens, ensuring cost‑effective medication therapy. Serve as a drug information expert, providing detailed drug information and education to the healthcare team, patients, and community. Review and evaluate medication orders for appropriateness and completeness, considering patient data such as age, weight, diagnosis, allergies, and laboratory values. Apply principles of continuous quality improvement, integrating quality improvement practices into activities and participating in drug use evaluations and medication safety oversight. Participate in local and system‑based committees, communicate with stakeholders, and develop innovative drug‑related patient care practices at institutional and system levels. Provide mentorship and education to other pharmacists, students, residents, and health‑care providers, and serve as preceptor or program director as required. Perform all functions in accordance with established policies, procedures, regulations, accreditation requirements, and professional standards, consistently demonstrating core and leader behaviors. Minimum Qualifications Doctorate in Pharmacy (PharmD) and completion of a PGY1 residency program, OR a minimum of 3years of progressive health‑system clinical pharmacy experience if residency training is not completed. Board certification through the Board of Pharmaceutical Specialties (BCPS, BCOP) that is active and maintained. Pharmacist licensure in the state of practice; if not initially licensed, must complete the Board of Pharmacy application process within 30 days of job offer and obtain licensure within 90 days of hire. Valid driver’s license and eligibility for coverage under the company’s auto insurance policy. ASHP Investigational Drug Service Certificate will be accepted for pharmacists providing care in Investigational Drug Services. Proficiency with pharmacy system software, Microsoft applications, and automated dispensing systems. Preferred Qualifications Completion of a PGY2 residency, or a PGY1 residency plus a minimum of 3years of health‑system experience. Board certification in an appropriate area of specialty. Expert level working knowledge of principles, practices, and operations in the assigned area of responsibility. Additional related education and/or experience preferred.
