Hospice Licensed Vocational Nurse (LVN) - Full-Time

Reporting to the MD of Hospice, the LVN provides skilled nursing care under the supervision of a Registered Nurse, delivering compassionate hospice services in patients' homes and care facilities. You'll support care plan implementation, collaborate with the interdisciplinary team, and help ensure every patient receives high-quality, patient-centered care.

What You'll Do

Key Responsibilities

• Deliver skilled nursing carefollowing directions of an RN & the patient's plan of care
• Assist the RN with carecoordination and interdisciplinary collaboration
• Educate patients and families oncare plans and self-care
• Monitor patient conditions andreport changes promptly
• Maintain accurate clinicaldocumentation
• Support infection control andregulatory compliance
• Assist with end-of-life care andpatient advocacy
• Promote quality care and agencyvalues
• Perform other duties as assigned

What We're Looking For

Qualifications

• Graduate of an accreditedVocational Nursing program
• Active Texas LVN license andcurrent CPR certification
• 2+ years of clinical LVN experiencerequired; Hospice or home health experience preferred
• Valid Texas driver's license,reliable transportation, and current auto insurance

Additional Requirements

• Ability to stand, walk, lift, up to50 lbs and safely transfer patients as needed
• Ability to travel locallythroughout the assigned service area
• Compassion, professionalism, adaptability& ability to work independently and as part of team
• Strong communication, team-work, criticalthinking & problem-solving skills
• Flexible schedule with PRNemergency call participation
• Ability to work effectively infast-paced, emotionally demanding environments
• Must pass a background check andprovide TB and Hepatitis documentation
