Location 1500 North Graham Street,Charlotte, NC, 28206,United States Base Pay $75,000.00 / Year Employee Type FT Exempt Manage Others No Nurse Case Manager Direct Reports: None Description The Case Manager provides centralized clinical case management, documentation oversight, and client support for OccuMedX occupational health programs across multiple construction and industrial sites. The role manages occupational injury and illness cases from initial notification through closure; coordinates timely follow-up and return-to-work activity; reviews clinical documentation for quality and completeness; and serves as a clinical resource to onsite medical personnel, Operations, Human Resources, leadership, and client stakeholders. The role also supports Fit for Duty services, employee exposure management, clinical quality initiatives, and injury data/trend reporting in accordance with applicable law, client requirements, OccuMedX protocols, and the individual's scope of practice. Key Responsibilities Injury Case Management & Oversight Manage occupational injury and illness cases for client workforces and internal employees from initial notification through case closure. Establish and document timely case follow-up, including worker contact, treatment status, referrals, restrictions, appointments, and return-to-work progress. Coordinate with onsite medical personnel promptly when a new injury, change in conditions, escalation, or other case-management concern is identified. Escalate complex, high-risk, delayed-recovery, or clinically concerning cases to leadership. Maintain clear case status, outstanding action items, and closure criteria so cases do not remain open without appropriate follow-up. Ensure timely follow-up, referrals, and return-to-work coordination in collaboration with clients and internal teams Monitor injury trends across client sites and internal operations, identifying opportunities for early intervention. Chart Review & Clinical Quality Review clinical charts, incident documentation, treatment notes, and follow-up entries for completeness, accuracy, timeliness, and alignment with OccuMedX clinical standards. Identify missing documentation, inconsistencies, or clinical concerns and communicate required corrections or follow-up to the clinical quality department. Identify recurring documentation or practice gaps that may require additional education and notify the clinical quality department. Support consistent documentation practices within the Company’s electronic health record and other approved clinical systems. Clinical Guidance & Escalation Serve as a clinical resource to onsite teams regarding injury management, exposure response, follow-up Collaborate with the Medical Director, supervising provider, or other appropriate clinician when a case exceeds the role's scope or requires provider-level medical decision-making. Promote early intervention and appropriate escalation while avoiding unnecessary delays in care. Post Offer Employment Test (POET) Program Oversight Coordinate and support client POET programs, including drug and alcohol testing and physical ability or functional testing in accordance with company policy, client requirements, and applicable federal and state regulations. Coordinate and perform drug and alcohol testing in accordance with company policy, client requirements, and applicable federal and state regulations Administer and oversee physical ability and functional capacity testing to assess employee readiness for work Review POET results and provide recommendations regarding work status, restrictions, or clearance, within scope and policy Ensure proper documentation, chain-of-custody, confidentiality, and compliance for all testing activities Serve as a professional clinical point of contact for assigned client stakeholders regarding case status, occupational health workflows, and permitted work-status information. Participate in client case reviews, operational meetings, quality discussions, and program reviews as requested. Support onboarding of new client programs by helping establish injury-reporting, case-management, escalation, documentation, and Fit for Duty workflows. Communicate timely, accurate, and appropriate information while protecting confidential medical information. Internal Employee Health Management Coordinate internal employee occupational injury and exposure follow-up in partnership with Human Resources and Operations, as appropriate. Support bloodborne pathogen exposure response, including timely post-exposure evaluation, documentation, and follow-up. Coordinate and track Hepatitis B vaccination offers, declinations, and related compliance documentation when assigned. Maintain employee medical information separately and confidentiality in accordance with applicable privacy, OSHA, and Company requirements. Compliance & Reporting Ensure compliance with OSHA, DOT (when applicable), state regulations, and client-specific requirements. Provide clinical information and documentation needed to support employer OSHA recordkeeping and recordability determinations without independently making legal determinations outside assigned authority. Track case-management metrics, follow-up completion, case aging, injury trends, escalation patterns, and other clinical quality indicators. Assist with audits, investigations, and internal or client-facing reporting. Serve as a liaison among onsite medical personnel, operations, leadership, and client representatives. Participate in clinical process improvement initiatives and help standardize case-management practices across OccuMedX sites. Support orientation and ongoing education of onsite medical personnel regarding case-management expectations, escalation pathways, and documentation standards. Maintain reliable availability and responsiveness during assigned working hours, including time-sensitive case escalations and client needs. Communicate case-related and POET findings professionally and in a timely manner. Qualifications Active clinical license (RN or equivalent) required Minimum 3–5 years of clinical experience, preferably in occupational health, emergency medicine, urgent care, or onsite medical services required Experience with injury case management, chart review, and clinical documentation preferred Experience performing or overseeing drug testing and physical ability testing preferred Knowledge of OSHA, bloodborne pathogen standards, and Fit for Duty program requirements preferred Strong written and verbal communication skills, including client-facing communication required Case management or occupational health certification (CCM, COHN, COHN-S, or similar) preferred Experience supporting construction, industrial, or multi-site client environments preferred Familiarity with electronic health record (EHR) systems required Strong clinical judgment, critical thinking, and attention to detail. Exceptional follow-through and ability to manage multiple active cases, deadlines, and competing priorities. Ability to recognize clinical risk and elevate concerns promptly. Strong documentation, auditing, coaching, and professional communication skills. Client-focused, solutions-oriented approach with the ability to balance clinical quality, compliance, and operational needs. Ability to work effectively across departments and with geographically dispersed onsite teams. Commitment to confidentiality and regulatory compliance Physical & Work Environment Office-based with occasional travel to client sites as needed Prolonged periods of computer use, chart review, telephone/video communication, and documentation.
