Transition Specialist

Grant-Funded Student Transition Specialist The Student Transition Specialist plays a critical role in supporting students as they transition from high school to postsecondary education or the workforce. This position provides comprehensive advising, coaching, and resource coordination to help students define and achieve their academic and career goals. The Student Transition Specialist fosters student success through individualized guidance, group programming, and collaboration with campus and community partners to remove barriers and promote equitable outcomes. Position Summary Responsible for providing support and guidance to students as they transition from high school to college or the workforce. Essential Duties and Responsibilities Include Provide individual and group advising to students on academic and career planning. Works with students to develop their academic and career goals, identify resources, and support services, and navigate the college or workforce application process. Facilitate workshops and presentations on college and career readiness. Work with students to identify and apply for financial aid. Coordinate with other campus departments and community agencies to provide support services to students. Maintain accurate records of student progress Report on program effectiveness to stakeholders Education and/or Work Experience Required Bachelor’s degree in education, counseling, social work, or a related field, 2+ years of experience working with students in a college or career readiness setting. Preferred Master’s degree in education, counseling, social work, or a related field. Other Skills and Abilities Strong interpersonal and communication skills. Ability to work independently and as part of a team. Ability to work with diverse populations. Proficiency in Microsoft Office Suite. Knowledge of working with students from disadvantaged backgrounds Experience working with students with disabilities. Experience working with students from diverse cultural backgrounds Training in trauma-informed practices.
