Job Summary:

We are seeking a highly skilled and compassionate Advanced Practice Registered Nurse (APN) to join our Skilled Nursing team at Avir at Camp Wood. As an APN, you will play a vital role in delivering high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure holistic care and exceptional outcomes.

Responsibilities:

• Provide advanced nursing care to residents, including assessment, planning, implementation, and evaluation of care plans

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Conduct resident assessments, perform skin checks, and provide wound care as needed

• Order and interpret lab results, as well as manage medications and treatments

• Develop and maintain relationships with residents, families, and caregivers

• Participate in quality improvement initiatives and maintain accurate records and documentation

Requirements:

• Master's Degree in Nursing (MSN) or equivalent

• Advanced Practice Registered Nurse (APN) certification

• Minimum of [n/a] years of experience as an APN

• Current RN license in the state of Texas

• Strong clinical skills and knowledge
