CenterWell Senior Primary Care, a subsidiary of Humana Inc., is looking for a Primary Care Nurse Practitioner or Physician Assistant. You will work onsite in one of the 8 primary care centers in The Villages, FL and report to the Physician Lead. You will enjoy a team-based practice model, manageable patient panels, and a stable Monday-Friday 8:00am-5:00pm outpatient schedule with no nights, weekends, or hospital call.

Responsibilities:

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Work with interdisciplinary teams to ensure holistic care.

Required Qualifications:

• Active, unrestricted Nurse Practitioner license in FL, or ability to obtain.
• Current national certification (AANP or ANCC).
• Active, unrestricted Physician Assistant license in FL, or ability to obtain.
• Successful completion of physician assistant practitioner training and certification as a practitioner (general or specialized) from an approved physician assistant program.
• DEA license (or ability to obtain).
• This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Preferred Qualifications:

• Experience in primary care, family medicine, internal medicine, or geriatrics.
• Comfort working with Medicare and managed care populations.
• Strong communication skills and a patient-focused approach to care.

Scheduled Weekly Hours: 40

Pay Range: $123,800 - $170,300 per year
