The Epilepsy Center of Northwest Ohio is seeking a Community Nursing & Residential Supervisor (RN or LPN) to support individuals receiving residential and waiver services.This role combines nursing and leadership, including healthcare coordination, staff training, compliance oversight, and supervision of a small caseload of waiver homes.If you're a licensed nurse who enjoys leadership and person-centered care, this is an opportunity to make a meaningful impact in the lives of individuals with developmental disabilities.

Residential Leadership:

• Supervise non-24 hour residential waiver homes.
• Lead, coach, and support Direct Support Professionals (DSPs).
• Coordinate staffing, scheduling, and daily operations.
• Ensure a safe, welcoming, and person-centered home environment.
• Support individuals in achieving greater independence and community inclusion.
• Participate in an on-call rotation and provide support as needed.
• Maintain compliance with agency policies, DODD requirements, and licensing standards.

Nursing & Clinical Oversight:

• Monitor the health and well-being of individuals served.
• Coordinate appointments, follow-up care, and communication with physicians and healthcare providers.
• Review documentation, MARs, and health records to ensure accuracy and compliance.
• Serve as the primary liaison with pharmacies for medication ordering and coordination.
• Monitor medication inventories and oversee prescription updates in eMARs.
• Investigate medication errors and implement corrective action and retraining when needed.

Training & Staff Development:

• Facilitate CPR/First Aid, Medical Orientation, and CaraSolva training.
• Complete medication pass observations and annual reviews.
• Mentor and develop staff through coaching, education, and performance management.

Care Coordination & Communication:

• Communicate regularly with guardians, physicians, hospitals, pharmacies, and agency leadership.
• Assist with healthcare planning and coordination of services to support individual health and quality-of-life goals.

Required Qualifications:

• Current, unrestricted RN or LPN license .
• Experience in developmental disabilities, healthcare, human services, residential services, or a related field.
• Strong leadership, communication, and organizational skills.
• Valid driver's license and acceptable driving record.
• Ability to pass background checks and required screenings.

Preferred Qualifications:

• Supervisory or management experience.
• Experience training or mentoring staff.
• Experience with electronic MAR systems, CaraSolva or similar software.
• Knowledge of DODD regulations and waiver services.
