Occupational Therapist

Full-Time (30-hours) Occupational Therapist (OT) We are seeking a skilled and compassionate Occupational Therapist to join our rehabilitation team at Capstone Center for Rehabilitation and Nursing, a 120 bed facility in Amsterdam NY. You will collaborate with an in-house interdisciplinary team that respects your clinical expertise to help residents regain independence in daily living and improve their overall quality of life Occupational Therapist Duties: Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans aligned with resident goals and physician orders Provide skilled OT treatment including but not limited to ADL retraining, environmental modification recommendations, adaptive equipment recommendations, and caregiver education Document all patient care, progress notes, and discharge summaries accurately and in compliance with Medicare/Medicaid and facility standards Supervise Occupational Therapy Assistants (OTAs) and rehabilitation aides as applicable, providing clinical direction and co-treatment as needed Participate in care conferences, MDS assessments, discharge planning, and quality improvement initiatives Maintain compliance with all state, federal, and accreditation regulations governing OT practice in long-term care settings Occupational Therapist Skills/Requirements: Degree in occupational therapy from an accredited program Current, unrestricted NYS OT license in good standing NBCOT certification (OTR) required SNF or long-term care experience preferred but not required- New Grads welcome.
