Clinical Pharmacist II
Cherry Hill
Role: Clinical Pharmacist II with a focus on acute internal medicine, providing comprehensive pharmaceutical care to hospitalized patients. Responsibilities Optimize medication therapy and support healthcare team rounding. Verify medication orders and perform pharmacokinetic dosing. Participate in antimicrobial stewardship and transitions-of-care counseling. Collaborate with physicians, nurses, and case managers to ensure safe, evidence-based prescribing. Precept learners and assist in protocol development. Contribute to quality improvement and medication safety initiatives. Qualifications & Skills Strong knowledge of pharmacology, medication management, and patient safety. Excellent collaboration and communication skills within a multidisciplinary team. Ability to support clinical decision-making and contribute to protocol development. Commitment to providing patient-centered care and continuous improvement. #J-18808-Ljbffr
