Continuum Therapy Partners is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team at Monroe Health Services in Monroe, Wisconsin. We have Full-Time and Part-Time opportunities available, with PRN positions also available for qualified candidates.

As a Speech-Language Pathologist, you’ll work with adult and geriatric patients in a skilled nursing setting, helping individuals improve communication, cognition, and swallowing function while enhancing their quality of life. Whether you’re an experienced clinician or a new graduate, you’ll be supported by a collaborative team committed to clinical excellence and outstanding patient care.

Responsibilities

• Evaluate, assess, and treat speech, language, cognitive, communication, and swallowing disorders.
• Develop individualized treatment plans based on each patient’s goals and needs.
• Collaborate with physicians, nursing staff, rehabilitation professionals, and family members.
• Complete timely and accurate documentation in accordance with state and federal regulations.
• Educate patients, caregivers, and staff regarding treatment recommendations and home programs.
• Maintain high standards of ethical, patient-centered care.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current Wisconsin Speech-Language Pathologist license required.
• Certificate of Clinical Competence (CCC-SLP) from ASHA required.
• Skilled nursing or rehabilitation experience preferred but not required.
• Clinical Fellows (CFs) and new graduates are encouraged to apply where applicable.
• Strong communication, organization, and teamwork skills.
