Sign-on Bonus: PT In-home Clinician (NP or PA) - Bellingham

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in‑home health evaluations. You will visit patients in their homes to provide comprehensive health assessments, including medication and medical history reviews, physical evaluations, and optional diagnostic tests such as diabetic eye exams, spirometry, and more. Responsibilities Conduct in‑home evaluations lasting approximately 45 minutes, connecting one‑on‑one with patients. Use an iPad and an intuitive clinical workflow for each assessment. Provide all Signify Health services as indicated, including diagnostic and preventive services (e.g., spirometry, DEE, bone density screening). Maintain flexibility to travel locally and within licensed states as needed. Conduct virtual visits when required. Participate in a national network of clinicians dedicated to improving health outcomes. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Ability to perform duties in diverse home environments that may contain pets, secondhand smoke, or allergens. Compensation & Hours Anticipated weekly hours: 29.5 Pay range: $46.03 - $99.14 per hour (base pay for each evaluation). The number of evaluations completed per day may vary. The position is eligible for additional reimbursement for diagnostic and preventative services performed.
