Physical Therapist Home Health

Provide physical therapy services to patients in their place of residence. Patients may have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with the patient, family, physician, and other allied health professionals. Identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation. Engage in interdisciplinary communications in accordance with agency policies. Follow procedures and standards of practice for home care services.
