Conviva Senior Primary Care, part of Humana, is launching a brand-new clinic in Port Salerno, Florida. As a Primary Care Physician you will provide hands-on, patient-centered care for seniors, in an onsite setting at the Conviva Delray Beach Center in Delray Beach. The role emphasizes longer 45-minute visits, a manageable daily panel of 14–17 patients, and a focus on social determinants of health.
