Job Summary

We are seeking an experienced Advanced Registered Nurse Practitioner (ARNP) to join a collaborative, patient-centered primary care team in Toledo, WA. This opportunity is ideal for a licensed Washington State ARNP with broad primary care experience who is passionate about evidence-based medicine, preventive care, and improving patient outcomes.

Responsibilities

• Conduct comprehensive patient assessments and diagnose and manage acute and chronic conditions within the ARNP scope of practice.
• Develop individualized treatment plans and provide patient education on preventive care, wellness, and disease management.
• Collaborate with physicians, nurse practitioners, physician assistants, and nursing staff to coordinate patient care.
• Exercise independent clinical judgment and apply evidence-based treatment approaches.
• Maintain accurate and timely clinical documentation.
• Complete appropriate billing documentation and diagnosis and procedure coding.
• Participate in daily care coordination meetings and interdisciplinary team collaboration.
• Monitor high-risk patients and support population health initiatives.
• Provide patient education, self-management guidance, and discharge planning.
• Participate in quality improvement, peer review, and clinical practice development.
• Support the clinical education of healthcare professionals and student nurse practitioners.
• Complete required annual compliance and safety training.

Required Qualifications

• Graduate of an accredited ARNP program.
• Current, active Washington State RN and ARNP licenses.
• Prescriptive authority in Washington State.
• Experience in a broad-based primary care clinic setting.
• Strong clinical assessment, diagnostic reasoning, and patient management skills.
• Excellent communication, documentation, and teamwork abilities.

Preferred Qualifications

• Experience performing minor surgical procedures or techniques.
• Experience with team-based primary care and population health management.
