We are currently looking for a compassionate organized and team-oriented Full-Time Home Health LVN to serve patients throughout San this essential role you will collaborate closely with our Director of Nursing clinical staff and office team to ensure coordinated high-quality care.

Responsibilities

• Implement the plan of care initiated by the Registered Nurse

• Provide accurate and timely documentation consistent with the plan of care

• Assess and educate patients and caregivers regarding diagnosis care plans and treatments

• Participate in coordination of home health services reporting additional needs (HHA PT OT ST MSW) to the RN or Clinical Supervisor

• Use equipment and supplies effectively and responsibly

• Complete online charting in Kinnser using company-provided devices

• Follow all company policies and procedures

• Perform other duties as assigned

Requirements

• Minimum 1 year of nursing experience in a medical setting

• IV certification and experience with wound care

• Home health experience recommended but not required

• Active California LVN license

• Valid California Drivers License

Job Type: Full-Time

Pay: $93000 - $115000/year DOE
