Registered Nurse (RN) Case ManagerSeeking an experienced and compassionate Registered Nurse (RN) Case Manager to join its team in Huntingdon, Tennessee. This position provides skilled hospice nursing care to patients facing life-limiting illnesses while supporting their families through education, care coordination, and symptom management. The ideal candidate will have prior hospice case management experience, strong clinical skills, and the ability to thrive in a collaborative, fast-paced environment with a growing patient census.Schedule & Work DetailsAgency: Baptist Hospice Huntingdon, TNSchedule: Monday through Friday, 8:00 AM to 4:30 PMWeekly Hours: 40 hoursPay Rate: $51.00 - $53.00 per hourProductivity Expectation: RequiredOn-Call Requirement: Shared rotation for weeknight and weekend on-call coverage among all case managersMileage Reimbursement: Yes, reimbursed between patient visitsKey ResponsibilitiesServe as the primary RN Case Manager for an assigned hospice patient caseload.Provide skilled nursing assessments, symptom management, and end-of-life care in accordance with hospice standards and physician orders.Develop, implement, and update individualized plans of care based on patient needs and goals.Coordinate care with physicians, hospice aides, social workers, chaplains, and other interdisciplinary team members.Monitor patient conditions and communicate changes promptly to the appropriate members of the care team.Provide education and emotional support to patients and families regarding disease progression, medications, symptom management, and hospice services.Maintain accurate, timely, and compliant clinical documentation.Participate in interdisciplinary team meetings and care planning discussions.Support agency operations during periods of census growth and increased patient needs.Participate in the agency's rotating on-call schedule, including weekends and evenings as assigned.Promote patient comfort, dignity, quality of life, and compassionate end-of-life care.Required QualificationsActive Registered Nurse (RN) license.Previous hospice case management experience required.Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal abilities.Valid driver's license, reliable transportation, and proof of automobile insurance.Ability to work independently while effectively managing a field-based caseload.Preferred QualificationsProven ability to manage multiple priorities in a fast-paced hospice environment.Strong teamwork and collaboration skills.Friendly, compassionate, and patient-focused approach to care.Excellent organizational and multitasking abilities.Experience working within an interdisciplinary hospice team.Dress CodeBlack or Royal Blue scrubs preferred.Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, pet insurance and employee discounts with preferred vendors.
