Hospice RN Case Manager (FT/PRN, Home Visits)

Concho Hearts Hospice

Concho Hearts Hospice seeks a compassionate Registered Nurse (RN) to provide hospice care in Carrollton and surrounding areas. You will perform in-home visits, coordinate with the hospice team, and complete admissions and recertifications while supporting patients and families with comfort and dignity. Active Texas RN license is required; hospice experience is preferred but training is provided.
