Continuum Therapy Partners is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team providing coverage at Jerseyville Nursing and Rehab in Jerseyville, Illinois, and Montgomery Nursing and Rehab in Hillsboro, Illinois .

This full-time Speech-Language Pathologist opportunity offers the chance to provide exceptional speech therapy services in supportive, collaborative skilled nursing facilities where your clinical expertise makes a meaningful impact every day.

As an SLP , you will work with adult and geriatric patients to improve communication, cognition, swallowing function, and overall quality of life while collaborating with an interdisciplinary rehabilitation team focused on delivering outstanding patient care.

Responsibilities

• Evaluate and treat patients with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized speech-language therapy treatment plans.
• Provide skilled therapy interventions related to communication, cognition, and dysphagia management.
• Complete assessments and make recommendations regarding swallowing safety and functional communication.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Educate patients, families, and staff on treatment strategies and safety recommendations.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Deliver compassionate, evidence-based speech-language pathology services.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current Illinois Speech-Language Pathologist (SLP) license required.
• ASHA Certificate of Clinical Competence (CCC-SLP) required based on state regulations.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates and Clinical Fellows are encouraged to apply.
• Strong communication, organization, and teamwork skills.
