divh2Registered Nurse/h2pQualifications:/pulliGraduated of an accredited school of nursing and licensed by the state of NJ as a RN./liliCurrent CPR or certified within 90 days of hire/liliOther certifications may be required for other areas of the organization/li/ulpstrongPosition Summary:/strong/ppThe RN assumes responsibility for a specific unit or area within the organization to include assessment (under the supervision of the DON/ADON), medication distribution, treatment clinical documentation, and supervision of CNAs and other staff./ph3Job Responsibilities and Standards/h3olliUses dual identifiers to confirm resident/guest identity prior to medication administration and treatments./liliPractices SBAR (situation, background, assessment, recommendation), the preferred method of communication when interacting with members of healthcare community./liliEncourages resident/guest to be involved in their own care when possible./liliReceives full and detailed report from the outgoing nurses to ensure proper communication and necessary follow-up of residents/guests care./liliRecognizes and responds competently to changes in residents condition and documents appropriately./liliInitiates Plan of Care on admission. Reviews, evaluates updates plan interventions as needed./liliInteracts and partners with all multidisciplinary service teams, internal and external, to assure continuum of care./liliPractices Standard Precautions and adheres to infection prevention strategies./liliDemonstrates compliance to Residents Rights./liliRefers to Nursing/Administration/IV/Emergency Management Policy and Procedures (Public Folders, Hard Copy manual) to assure standard of care./liliTranscribes physician orders correctly on MAR TAR./liliFaxes POS to Pharmacy provider to ensure timely delivery and administration./liliMaintains a safe environment for residents/guests and reports unsafe situation for resolution./liliFollows chain of command./liliAdheres to proper hand washing technique./liliAdministers medications according to policy and procedure./liliNotifies responsible party of changes in patients condition./liliWorks closely with Social Worker in trying to resolve residents/guests/ needs./liliChecks syringes, and counts narcotics at end of shift./liliOrders stock medications and prescription medications; as needed./liliAdmits and discharges residents/guests according to facility procedure./liliReviews report with CNAs prior to their assignment activation./liliPerforms assessments related to:/liulliSkin/liliCardiac/liliRespiratory/liliGI/liliNeurological/liliMuscular Skeletal/liliRenal/liliPain/li/ulliAccurately documents on the following forms:/liulliAmbulation/Restorative care/liliPsychotropic Monitoring/liliPain Management/liliPressure Ulcer Management/skin assessment/liliFall Prevention Strategies/liliAntibiotic documentation/liliIncidents/accident/liliNurses Note day 1-5 and day 6 -Discharge Documentation/li/ulliAccurate and timely completion of Nursing assessment and data collection upon admission:/liulliFall Risk assessment/liliSide Rail screen/liliBraden Scale/liliBowel and Bladder/liliElopement Risk Profile/liliPain Evaluation/liliSmoking Evaluation/liliData collection/liliImmunization Record Sheet/li/ulliContacts physician for admission orders:/liulliReconciles medications/liliDocuments legibly onto POS/liliRead back and verifies Telephone Orders/liliMake appropriate referrals/liliTranscribes all orders./liliFax back (new admissions)/li/ul/olh3Facility-Wide Responsibilities Standards/h3pstrongEmployee is required to:/strong/pulliSpeak English in patient care areas./liliAttend mandatory and scheduled in-services/meetings./liliArrive to work on time as scheduled./liliDemonstrate ability to work with others cooperatively./liliDemonstrate flexibility in assignment to meet resident/family needs./liliWear ID at all times appropriately when working./li/ul/div
