Pharmacy Technician Medication History

Purpose: Under the supervision of a pharmacist, the medication history technician (MHT) will obtain medication histories through interviews with the patient/family/care partner. The MHT will use technology to pull in external fill history from patients� pharmacies. Essential Responsibilities: Obtains and clarifies patient�s home medication list, including prescription, OTC, and herbal products. Any bottles brought in will be promptly returned to the patient�s family/care partner after all important information has been transcribed. Responds to nurses, physicians, pharmacists, or other requests to manage, triage appropriately, and ensure follow-up promptly regarding med history information. Obtains patient�s allergies & intolerances and communicates to appropriate caregiver for proper documentation into the electronic medical record (EMR), including symptoms, reactions, and severity. Directs, triages, and communicates to appropriate caregiver all questions and inquiries from patient/family/care partner outside the scope of the MHT. Identifies and participates in ongoing quality assurance and improvement processes to obtain an appropriate med history list. Fulfills all compliance responsibilities related to the position. Performs other duties as required Required: High School Diploma or equivalent Minimum Experience: 1 year Desired: Retail Pharmacy Technician experience. Computer skills, ability to multi-task and work independently. License, Registration, or Certification Requirements: PTCB Certification within 6 months License: Registered Pharmacy Technician with NY Board of Pharmacy within 6 months Other Information: Working Condition: Communicates clearly in-person / over the phone / respond to questions.
