Behavioral Health Case Manager — Integrated Care (Part-Time)

Care Plus NJ, Inc.

CarePlus NJ Inc is seeking a Case Manager to join our Behavioral Health Home in Paramus, NJ. This on-site, part-time role (20 hours) focuses on coordinating care for adults with serious mental illness and chronic medical conditions to improve overall health outcomes. You will lead psycho-educational groups, provide on-site and outreach services, and collaborate with a multi-disciplinary team to develop and implement comprehensive care plans using evidence-based approaches. #J-18808-Ljbffr Care Plus NJ, Inc.
