Occupational Therapist (OT) - Skilled Nursing Facilities

Key Responsibilities:
Develop and implement individualized rehabilitation plans to help patients achieve their goals and regain independence
Conduct thorough assessments to identify and address physical, cognitive, and emotional needs
Collaborate with interdisciplinary teams, including nursing, physical therapy, and speech therapy
Design and implement adaptive equipment and home modifications to enhance patient safety and functionality
Provide education and support to patients, families, and caregivers on coping strategies and wound care management
Document patient progress and communicate effectively with healthcare providers
