Department Scotland Memorial Hospital - Transition and Navigation Services Employment Type Full time Shift 1st (United States of America) Schedule Details & Additional Information Experience: Minimum requirement is at least 1 year of professional experience in a hospital or health-related setting (preferred). Expertise: Knowledge of data management tools. Adherence to the National Association of Social Workers Code of Ethics. Responsibilities Interview patients and their social support systems to complete psychosocial evaluation/assessment. Provide counseling, support, and education to patients and their support systems to develop healthy coping strategies and improve overall well-being. Collaborate with patients and primary care providers to establish and implement a plan of care that meets patients' psychosocial needs. Document patient visits and follow-up per standard guidelines, track appointment attendance, and close care gaps to advance wellness. Serve as a resource to SPN regarding emotional, social, and psychosocial components of illness and assist in a care team approach with primary care providers. Intervene in crisis situations applying crisis intervention skills appropriately and effectively. Work with SPN team to assess and evaluate patients' and support systems for appropriate level of care or environment to meet needs. Participate in care planning meetings as requested. Maintain up-to-date knowledge of community agencies and resources for social, emotional, or financial assistance. Round with each provider daily in clinic to build referral relationships and provide warm patient handoffs; ensure clinic managers and providers know the availability and hours on site for acute intervention. Provide education to patients and supporters on community resources and options appropriate to patients' ages; make referrals to community agencies as needed. Report suspected child and adult abuse/neglect/exploitation cases; serve as liaison between practice, patients/support systems, and Department of Social Services during evaluation and investigation. Identify and record clinical, psychosocial, and financial barriers for smooth transition across the healthcare continuum and facilitate system improvements. Work with SPN leadership to identify problems, recommend solutions, and collaborate toward resolution. Develop and maintain accurate clinical records in EPIC for each referred patient, documenting per departmental standards. Collaborate with SPN leadership to build an LCSW integrated primary care program. Qualifications At least 1 year of professional experience in a hospital or health-related setting.
