Field RN Care Manager: CKD/ESKD Home & Clinic Visits

Somatus is seeking an RN Care Manager to support high-needs CKD/ESKD patients, working with physicians and care teams to improve outcomes. This role involves in-home and facility visits, care planning, and strong collaboration with nephrology practices within designated markets. The position requires an active RN license, 2+ years of RN experience in a multidisciplinary setting, and willingness to travel.
