The Enclave at Rye Rehabilitation & Nursing Center is seeking a dedicated and experienced Clinical Nutrition Manager (Registered Dietitian) to oversee our nutrition program and support the delivery of high-quality dietary care to our residents.

The Clinical Nutrition Manager will work closely with nursing leadership, dietary services, and the interdisciplinary team to ensure residents receive individualized nutrition support that enhances their overall health and quality of life.

Responsibilities

• Lead and oversee the facility’s clinical nutrition services and dietary care program.

• Perform comprehensive nutrition assessments and develop individualized nutrition interventions based on resident needs.

• Evaluate weight changes, nutritional risks, laboratory results, and overall nutrition status.

• Develop and monitor therapeutic diet plans, nutritional supplements, and specialized nutrition recommendations.

• Collaborate with nursing, physicians, rehabilitation, speech therapy, and other members of the interdisciplinary team.

• Participate in resident care plan meetings and provide clinical nutrition recommendations.

• Support and educate dietary team members on nutrition standards, compliance, and best practices.

• Monitor dietary operations to ensure meals are safe, nutritious, appealing, and aligned with resident preferences.

• Ensure compliance with CMS guidelines, state regulations, and facility policies related to nutrition services.

• Conduct quality audits and assist with quality improvement initiatives.

• Provide nutrition education to residents, families, and staff.

• Maintain accurate clinical documentation and support survey readiness.

Qualifications

• Current Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR).

• Active New York State licensure or eligibility to practice as required.

• Previous experience in a Skilled Nursing Facility, Long-Term Care, Rehabilitation Center, or healthcare setting preferred.

• Leadership or supervisory experience within clinical nutrition or dietary services preferred.

• Strong understanding of clinical nutrition, therapeutic diets, and regulatory requirements.

• Excellent communication, organization, and leadership abilities.

• Ability to work independently while effectively collaborating with an interdisciplinary team.
