Join Our Dynamic Team as a Physical Therapist Job Description: - Are you a passionate and skilled Physical Therapist looking to make a real difference in the lives of others? - Join our interdisciplinary team in a vibrant Skilled Nursing Facility (SNF) where your expertise will shine! - Your role will include: - Performing thorough assessments of patients’ physical abilities, movement limitations, and pain levels. - Developing and implementing personalized treatment plans based on evaluation findings, medical history, and patient goals. - Delivering evidence-based physical therapy interventions to enhance strength, coordination, balance, and mobility. - Collaborating with physicians, nursing staff, occupational and speech therapists, and family members to deliver comprehensive patient care. - Monitoring patient progress, adjusting therapy plans as needed, and documenting all care provided in accordance with state and federal guidelines. - Educating patients and caregivers on safe mobility techniques, home exercises, and equipment use. - Ensuring compliance with all regulatory requirements, including Medicare/Medicaid documentation and facility policies. - Participating in care planning meetings and interdisciplinary team conferences. - Supervising physical therapist assistants (PTAs) and aides as necessary. - Maintaining a safe and clean work environment. Qualifications: - Graduate of an accredited Physical Therapy program. - Current state license to practice as a Physical Therapist (PT). - CPR certification (preferred or required depending on the facility). - Experience in a Skilled Nursing Facility or geriatric rehab setting preferred. - Excellent communication, documentation, and interpersonal skills. - Ability to work independently and collaboratively within a multidisciplinary team. Work Environment: - Skilled Nursing Facility (SNF) focused on inpatient, long-term care, and short-term rehabilitation. - Involves lifting, standing, walking, and assisting patients with transfers and mobility.
