Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Experience in home health is not required

Assignment Details

• Schedule: Monday through Friday
• EMR: HCHB
• Territory: El Paso County
• Projected mileage: 200 miles per week
• Works with a PTA in a care coordination model
