Hospice Social Worker

Three Oaks Hospice – Social Worker Three Oaks Hospice provides compassionate palliative care to patients and families. We are looking for a Social Worker to deliver coordinated care and psychosocial support in patients’ homes. Position Summary Primary function is to provide coordinated care in the homes to patients of all age groups and perform psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient/family. Responsibilities Assess and complete the Comprehensive Assessment within five days of the patient’s election of hospice care to identify patient and family’s psychosocial, financial, environmental and community needs as evidenced by POC documentation, clinical records, IDG/IDT meetings and community resource referrals. Assist with advance directives, funeral planning, discharge planning, family counseling and may assist in changes in the level of care as indicated by the patient or family’s needs. Use effective interpersonal communication skills, both verbal and written. Meet mandatory continuing education requirements of the Hospice and licensing board. Maintain a working knowledge of public and private eligibility standards and requirements. Provide patient and family/caregiver counseling around issues of end‑of‑life, death, and grief. Promote hospice philosophy and administrative policies to ensure quality of care. Attend IDG/IDT meetings, participate in the patient care planning process and coordinate and collaborate with the IDG/IDT to promote coordination of patient care. Accept clinical assignments that are consistent with education and competence to meet the needs of the patients. Provide an environment that promotes respect for patients and their privacy and property. Participate in the Hospice’s QAPI program. Follow the Hospice infection control policy. Participate in the Hospice’s educational in‑service programs as required. Qualifications Bachelor’s or master’s degree in social work from a state‑accredited program. If holder of a bachelor’s degree and hired after 12/02/2008, must be supervised by a Master of Social Work. Current and valid license as an LMSW. Working knowledge of community resources. Good interpersonal skills. Current state certification. Reliable transportation, valid driver’s license and current auto liability insurance. #J-18808-Ljbffr
