Job Summary and Responsibilities The Social Worker (MSW) is responsible for providing non‑clinical, biopsychosocial assessments and interventions across intensive care units, trauma, oncology, NICU, transplant, and the ED. Key tasks include crisis intervention, patient/family support, high‑risk screening, referrals for financial assistance, post‑acute care placement, and coordination with the multidisciplinary team. Responsibilities Collaborate with RN case managers, nurses, physicians, insurance companies, and other staff to proactively identify and address patient needs. Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual barriers. Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed‑care concepts related to length‑of‑stay management. Perform thorough assessments and develop appropriate discharge plans for high‑risk patients. Coordinate and facilitate meetings between the care team, patients, families, and caregivers to honor their preferences and values. Provide psychosocial support, education, counseling, advocacy, and referrals to address emotional, social, and financial aspects of care. Offer emotional support during complex or emotionally charged situations such as end‑of‑life care and crisis interventions. Qualifications Master’s Degree No Travel Required. No Experience Required.
