A-Line Staffing is hiring an  RN Case Manager for an  onsite contract-to-hire opportunity in  Grand Junction, CO . This role focuses on assessing high-risk members, coordinating care across providers, and ensuring safe, cost-effective care transitions.

Schedule / Details

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Hours: 40 hours/week (40 guaranteed)
• On-call: Potentially 1 day/week and 1 weekend/month (not guaranteed)
• Work setting: Onsite
• Counties covered: Mesa County
• Mileage reimbursement: $0.43/mile

Key Responsibilities

• Perform comprehensive assessments of  high-risk members
• Partner with primary care providers to build and execute  individualized care plans
• Continuously reassess and direct members to  appropriate, cost-efficient levels of care
• Arrange and coordinate  care transitions with physicians, social workers, discharge planners, and other stakeholders
• Engage specialty resources as needed to resolve complex situations
• Track and document  clinical, functional, and fiscal outcomes
• Identify health promotion and prevention opportunities
• Advocate for patients while protecting  privacy and confidentiality

Required Qualifications

• Active, unrestricted Colorado RN license
• Associate’s or Bachelor’s degree in Nursing (or related field)
• Case Management certification
• Ability to analyze complex clinical information and make sound decisions
