As an Occupational Therapist (OT), you will have the opportunity to work with a diverse patient population and provide high-quality, patient-centered care in a supportive and collaborative environment. As a per diem Occupational Therapist (OT), you will work on an as-needed basis, allowing you to choose your own schedule and work at your own pace. About the Role: Provide occupational therapy services to patients in our skilled nursing and rehabilitation center, including assessments, interventions, and evaluations Develop and implement individualized treatment plans to help patients achieve their goals and improve their functional abilities Collaborate with other members of the healthcare team to ensure comprehensive care and effective communication Document patient progress and outcomes in accordance with regulatory requirements and professional standards Responsibilities: Conduct initial and ongoing assessments of patients' physical, emotional, and cognitive abilities Design and implement customized occupational therapy programs to address patients' specific needs and goals Provide education and training to patients, families, and caregivers on functional activities, adaptive equipment, and other rehabilitation techniques Maintain accurate and detailed records of patient treatment, progress, and outcomes Participate in interdisciplinary team meetings to discuss patient care and treatment plans Requirements: Bachelor's or Master's degree in Occupational Therapy from an accredited program Current OT licensure in the state of Wisconsin Strong communication and interpersonal skills Ability to work effectively in a fast-paced, dynamic environment Strong analytical and problem-solving skills Work Environment: Our friendly and welcoming facility is located in Fort Atkinson, Wisconsin, offering a comfortable and supportive environment for patients and staff Our team is dedicated to providing exceptional care and compassion to our patients and their families Why Choose Rock River Nursing & Rehab? competitive compensation package opportunities for professional growth and development #J-18808-Ljbffr Rock River Nursing & Rehab
