Job Summary and Responsibilities This is a PRN position but will be included in after hours and weekend call rotations CommonSpirit Health at Home leverages AI-powered clinical documentation to reduce documentation, enhance work-life balance, and maximize patient time. - Collaborate and coordinate. Work seamlessly with a multidisciplinary team - physicians, therapists, and support staff - to ensure holistic care.
- Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
- Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
- Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
- Graduate of an accredited nursing program with an active RN license in the state served.
- Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
- Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver's license and reliable transportation.
