Overview
Licensed Practical Nurse (LPN) – Long-Term Care (LTC) – Malone, NY. Travel assignment starting 08/10/2026 for 6 weeks, evenings on a 5x8 schedule. Weekly pay ranges from $2,100 to $2,157 per week. No guaranteed hours. Candidates must have an LPN license on file at submission and LTC experience; the role requires coverage with a minimum of every other weekend and scheduling adjusted to unit needs in a long-term care setting.

Responsibilities
- Provide direct patient care to residents in a long-term care setting under supervision
- Administer medications and monitor effects according to the care plan; document interventions
- Collaborate with licensed staff and CNAs; cover CNA duties as needed to support patient care
- Monitor vital signs, assess for changes, and respond to clinical concerns promptly
- Perform routine nursing procedures and assist with activities of daily living; honor resident dignity and safety
- Document care in the electronic medical record and communicate updates to the care team; ensure infection control and safety practices

Requirements
- Active LPN license on file at submission (New York license preferred)
- AHA BLS certification required
- Minimum 1 year of LTC experience
- LPN may cover CNA duties as needed
- Graduation from an approved LPN program
- Must pass a 7-year background check (NY state, nationwide, county searches, FACIS3, and sex offender) within 18 months of start date; FACIS3 and NY state check within 30 days
- Must be able to start within the next week
