Clinical Manager (RN) – Home Health RN

[Hernando County, FL and some days to sit in OcalaOffice per month] | Full Time

As a Clinical Manager (RN), you will play a key leadership role in our home health operations. You will collaborate with referral sources, physicians, and community partners to coordinate patient care, ensure regulatory compliance, and support clinical excellence.This position oversees a multidisciplinary team of nurses and therapists, ensuring that every patient receives high-quality, compassionate care in the comfort of their home.

 

What You'll Do

• Lead and support a high-performing team of RNs, LPNs, therapists, and home health clinicians
• Oversee patient care planning, case management, and clinical outcomes
• Ensure compliance with home health regulations and accreditation standards
• Serve as a liaison with hospitals, physician practices, and referral partners
• Deliver exceptional customer service to patients, families, and community partners
• Maintain accurate documentation and utilize EMR systems effectively

What You'll Need

• Active Florida RN license
• Minimum one year of nursing experience
• Home health Leadership experience preferred
• OASIS experience preferred
• Strong assessment and critical thinking skills
• Ability to work independently
