Physical Therapist PRN Job Summary: Glenbridge Health and Rehabilitation of Boone, NC, is seeking a highly skilled and dedicated Physical Therapist (PT) to join our team on a PRN basis. As a valued member of our healthcare family, you will be responsible for providing exceptional physical therapy services to our residents, empowering them to achieve their highest level of physical function and independence. If you are passionate about delivering high-quality care and making a positive impact on the lives of others, Physical Therapist Key Responsibilities: Conduct comprehensive physical therapy evaluations and develop personalized care plans based on clinical findings and resident goals Deliver physical therapy treatments as prescribed, including therapeutic exercises, gait training, balance activities, and functional mobility Monitor and document residents’ progress, response to treatment, and adjust plans as necessary Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians to provide holistic care Educate residents, families, and caregivers on therapy programs, techniques, and safety measures to support functional independence Comply with all regulatory, licensing, and facility policies including documentation and infection control standards Physical Therapist Qualifications: Graduate of an accredited Physical Therapy program Current and valid Physical Therapist license CPR certification required (or must obtain upon hire) Prior experience in a skilled nursing or long-term care setting preferred Strong communication, clinical, and organizational skills Additional Requirements: Minimum of 1 year of experience as a Physical Therapist Ability to travel to and from work independently as needed Availability to work variable shifts, including days, evenings, and weekends Join Our Team: Glenbridge Health and Rehabilitation is committed to providing high-quality care to our residents and exceeding their expectations. #J-18808-Ljbffr Glenbridge Health and Rehabilitation
