Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for care plan development
• Address and reassess cost-efficient, appropriate levels of care

Requirements of the RN - Case Management

• Current, unrestricted MS RN license or willing to accept compact license
• Associate's or Bachelor's Degree in Nursing or related field
• Experience in Home Care, HCHB Oasis
• Case Management certification
• Ability to analyze complex medical information and make decisions
