West Virginia Licensed Clinical Pharmacist

To support a dynamic medication therapy management program, the full-time remote West Virginia Licensed Clinical Pharmacist will perform comprehensive medication reviews, drive medication interventions, and collaborate with clinical teams to optimize patient care. Key responsibilities Perform and write concise member-specific comprehensive medication reviews (CMRs) while adhering to productivity and quality standards Drive meaningful medication interventions to ensure adherence to evidence-based medication guidelines among medical providers and members Collaborate with clinical care teams and serve as a pharmacist resource regarding medication use and management resources Required qualifications Bachelor's degree in pharmacy or PharmD Current pharmacist license in West Virginia 3+ years of experience actively practicing as a pharmacist in a managed care, ambulatory care, or clinical practice environment
