RN or LPN – Community Nurse Liaison/Care Manager

Home Care Plus is seeking an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a strong background in geriatrics, non-medical home care, home health, hospice, senior living, or care management .

Please Note: This position requires previous experience in non-medical home care, home health, hospice, geriatric care, senior living, or care management. We are seeking an experienced professional who can step into this role with minimal training.

This is not a bedside nursing position.

We're looking for a seasoned healthcare professional who understands the unique needs of older adults, enjoys building lasting relationships, and is passionate about helping seniors remain safe and independent in their homes.

What You'll Do

• Conduct comprehensive in-home client assessments.
• Develop individualized care plans based on each client's needs and goals.
• Meet with prospective clients and families to educate them on available care options.
• Build and maintain professional relationships with hospitals, physician offices, rehabilitation centers, senior living communities, hospice organizations, social workers, case managers, and other referral partners.
• Represent Home Care Plus at networking events, healthcare meetings, and community outreach activities.
• Support caregiver recruitment, onboarding, orientation, and quality assurance visits.
• Provide clinical guidance and serve as a resource for clients, families, caregivers, and referral partners.
• Collaborate with leadership to ensure exceptional client satisfaction and continuity of care.

Qualifications Required

• Active South Carolina RN or LPN license in good standing.
• Minimum 5 years of experience in one or more of the following:
• Non-medical home care
• Home health
• Hospice
• Geriatric care
• Senior living
• Case management
• Care coordination
• Experience completing in-home assessments and developing care plans.
• Strong interpersonal, organizational, and communication skills.
• Ability to work independently and manage multiple priorities.
• Reliable transportation and willingness to travel throughout Charleston, Berkeley, and Dorchester Counties.

Preferred

• Existing relationships within the local healthcare community.
• Experience working with referral sources such as physicians, hospitals, rehabilitation centers, skilled nursing facilities, hospice agencies, or senior living communities.
• Business development or community outreach experience within healthcare.
• Experience recruiting, mentoring, or supervising caregivers.
