Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 Weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum Years of Experience Required License: NC RN License Required Certifications: BLS Description: Monday to Friday from 8:00 AM to 4:30 PM with a weekend rotation of 1 out of 4 weekends required. Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, responsible for providing skilled care and case management to patients and families in accordance with the State Nurse Practice Act and accepted professional standards of practice. The RN case manager provides services utilizing nursing theory and processes. Communicates and documents observations and assessments. Promotes and maintains health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.
