Evening Shift LPN
Job Summary
We are seeking a compassionate and skilled Day & Evening Shift Licensed Practical Nurse (LPN) to join our team at Sapphire Rehabilitation & Care Center in Columbus, Ohio, USA. As an Day & Evening Shift LPN, you will play a vital role in providing high-quality patient care to our residents, while also collaborating with our interdisciplinary team to ensure seamless transitions and exceptional outcomes. Responsibilities
- Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Conduct thorough assessments, identify areas of risk, and develop interventions to prevent complications
- Document patient information accurately and thoroughly in accordance with facility policies
- Maintain open communication with patients, families, and colleagues to ensure continuity of care
- Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction
- Demonstrate a commitment to patient-centered care, adhering to our facility's mission.

Requirements
- Current LPN licensure in the state of Ohio
- High school diploma or equivalent required; associate's degree in nursing preferred
