Substitute School Nurse
Minimum Qualifications: New Jersey Substitute School Nurse certificate and Registered Nurse license Active Substitute Nurses in Other Districts
1. Provide a valid NJ Substitute School Nurse Certificate and complete online application.
2. Complete the Criminal History Review process with the Office of Student Protection.
3. Provide Documentation of Mantoux test with date and results. Please provide your most recent Mantoux/TB result per guidance from the NJ DOE.
How to Apply for a New Nurse Substitute Certificate:
1. Get fingerprinted and print your approval certificate from the NJ Office of Student Protection website: Office of Student Protection Status (state.nj.us). If asked throughout the fingerprint scheduling process, use the following codes for Franklin Borough School in Sussex County New Jersey: County Code: 37 District Code: 1570 School Code: 060 Service Code: 2F1FB1 Contributor's Case #: 371570.
2. To apply for a substitute nurse credential (initial or renewal), you must create an account as a NEW APPLICANT in NJEdCert and follow directions for the substitute nurse application and uploading your criminal history certificate and nursing license.
